OUTREACH TEAM LEADERS (WBPHCOT PROGRAMME)- PROFESSIONAL NURSE (GENERAL NURSING)

Job Overview

The Northern Cape Department of Health is advertising 15 posts for Outreach Team Leaders (WBPHCOT Programme) - Professional Nurse (General Nursing) within the Namakwa District. Depending on experience, positions are available at Grade 1, Grade 2, and Grade 3 levels. Requirements include a basic R425 nursing qualification, valid SANC registration as a Professional Nurse, and a driver's license, alongside relevant years of post-registration nursing experience. Successful candidates will lead and manage community health outreach teams, provide clinical supervision, and ensure continuity of healthcare services. Requirements & Qualifications

• Basic R425 qualification (i.e. diploma/degree in nursing) or equivalent qualification that allows registration with the SANC as a Professional Nurse.
• Registration with the SANC as Professional Nurse.
• Driver’s license.
• Grade 1: None required.
• Grade 2: A minimum of 10 years appropriate/recognisable experience in nursing after registration as a Professional Nurse with the SANC in General Nursing.
• Grade 3: A minimum of 20 years appropriate/recognisable experience in nursing after registration as a Professional Nurse with the SANC in General Nursing.

Key Duties & Responsibilities

• Lead and manage the WBPHCOT to achieve defined population coverage and measurable improvements in community health through prevention, early identification, referral, linkage and follow-up.
• Provide direction and supervision for the implementation of the nursing plan (clinical practice/quality patient care).
• Implement standards, practices, criteria and indicators for quality nursing (quality of practice).
• Practice nursing and health care in accordance with the laws and regulations relevant to nursing and health care.
• Maintain a constructive working relationship with other health facilities, strengthening community based services.
• Ensure continuity of health care services to individuals and households.
• Provide management, supervision and oversight leadership for the WBPHOT.
• Perform monitoring and reporting of WBPHOT and provide guidance and expert support.
• Assist with achieving targets as identified by health facilities.
• Facilitate community mobilisation and create awareness on health diseases through awareness campaigns and mobilise around community needs.
• Record and report all activities as per reporting tools.
